- Big refactor to do structured logging more properly. Now for every request, a single message is logged with everything significant that happened.

- Fixed some bugs caused by new versions of joserfc being more strict about what can be used in a
sub claim.
- Fixed a bug that was causing production dependencies to get ahead of development.
